This form will accept proof of vaccination for all players and coaching staff over 12 years of age. This is a requirement by the OMHA Vaccination Policy for all OMHA sanctioned events including games, practices and tournaments. All Affected Persons are required to be fully vaccinated (including the 14-day period after receiving their second dose) by October 31st, 2021 or earlier. All affected persons must present to their to the OAAA Head Trainer, evidence satisfactory to the OMHA demonstrating that they have received the completed series of an accepted Covid-19 vaccine. ALL DOCUMENTS WILL BE REVIEWED AND THEN DELETED FROM OUR RECORDS.
