Proof of Vaccination for OAAA Players & Coaches

As per the direction from the OMHA, the OAAA is required to see proof of vaccination for all our players aged 12 and older as well as all bench staff and on-ice helpers in our programs.

We are trying to make this as easy for everyone involved.



We have added a Proof of Vaccination form to our website where you can upload your receipt as proof you meet the vaccination requirements.  

This receipt will only be seen by our Head Trainer.  She will check off that she has seen the receipt and then delete the receipt permanently from our files.  Posted at the arena are QR codes (similar to the contact tracing) which you can use to fill out the Proof of Vaccination form or you can click on the link below.

Everyone MUST show their proof of vaccination before October 31st.  Please do this as soon as possible to avoid any interruptions in our season.
